Not studying their previous fights enough to see what actually works and what gets him in trouble. Not training enough to remove bad habits and get good ones ingrained.

OUCH!
Why can't Pac realize that when his right foot is outside of Juan's lead foot, he can shoot his snap left lead and roll under safely ? Juan looked lost trying to spin to his left everytime Pac pulled the move. The flash knockdown is a classic example. Pac snaps the left then Juan trips on Pac's right foot.

Imagine him doing that for one more round while Juan's nose is busted.

You're precisely right!!! PAC fallen to Marquez trap, following Marquez on his right, it would be same result if there is match V, VI or VII or unitl Marquez turn 50, PAC would always loss if he keep following Marquez. Why Roach and the rest of Pacquiao team never saw this and advise Pacquiao to cut the ring and move to his right. For me Marquez really deserved the win, he was so determined, focus (even with blooded nose) and very composed to KO Pacquiao. Dream come true for him.

he lost because signing of cross is part of his focus and concentration before and after the rounds.
